APPROVAL APPLICATION

What are the activities that need approvals from AELB although the license to deal with the board has been approved?
Although license to deal with the board has been approved, the licensee has to get an approval from AELB to proceed activity such as:
Approval that requires an application form:
a. Approval application of the radioactive materials import/export/movement activity.
b. Approval application of the irradiating apparatus import/export activity.
c. Approval application of the radiation worker loan
d. Approval application of the radioactive materials loan
e. Approval application of the irradiating apparatus loan
f. Approval application for radiography work in public area
g. Approval application of the irradiating apparatus install / commission / maintenance 
Approval that needs a letter of intend:
Exchange/recruitment/termination of the OBTL, Radiation protection officer, supervisor, or radiation worker.
Construction of the storage room
Use the storage room
Disposal of the radioactive materials
Disposal of the irradiating apparatus
Relocation of irradiating apparatus

GENERAL REQUIREMENT FOR APPROVAL APPLICATION

What are the general requirements for approval application?
a. License still valid
b. Radiation Protection officer recognition period is still valid.
c. Radioactive materials or irradiating apparatus (include all related item) has been licensed and not exceed the licensed limit.
d. Licensee or worker is not in AELB investigation (To Whom It May Concern)
e. Application form should be completed (if using form)

IMPORT/EXPORT/MOVEMENT OF RADIOACTIVE MATERIALS

What are the procedures to import or export radioactive materials?
a. Obtain approval application form for import/export/movement of radioactive materials activity from AELB or print out form from AELB website at http://www.aelb.gov.my 
b. Include 3 copies of application letters inclusive RM20.00 fee for every application letter no later than 14 days before commencing activity. 
c. Upon receiving approval and start-up the activity, present related information such as in Attachment A which include approved application letter. 
d. Approval (if qualified) will only be given for this period, whichever is shorter:-
i. 3 months from approval date; or
ii. Until license valid date; or
iii. until RPO certificate valid date.

RADIATION WORKER LOAN

What are the procedures and the requirements to loan the radiation worker?
a. Requirements of loaning radiation worker are:
b. Valid license from AELB for borrower and lender.
c. Radiation Protection Officer still within approval limit.
d. Radiation worker involve:
i) Obtain recognition as radiation worker
ii) Recognition period still valid
iii) Not a Radiation Protection Officer
iv) Not within AELB investigation 
v) Not in the period of suspension
vi) Not receive accumulated dose (for current year) exceed operation limit.
e. Lender is not in the period of suspension of license/operation.
f. Obtain approval application for leaning radiation worker form from AELB at the price of RM 1.00.
g. Form should be given not less than 14 days from lending date.
h. Approval (if qualified) will only be given for this period; whichever is shorter:
i) Application period; or
ii) until lender license approved valid period; or
iii) Until borrower license approved valid period; or
iv) Until RPO lender recognized valid period; or
v) Until RPO borrower recognized valid period; or
vi) Until worker approved recognized valid period
Approval requirement is stated at the back side of the application form.

RADIOACTIVE MATERIALS LOAN

What are the requirement and procedures to loan the radioactive materials?
Loaning requirement of radioactive materials are:
a. total number to be loan and total number of existing that not exceed the total number licensed.
b. Loan accumulated period (if ever make a loan application for the same equipment) do not exceed one year.
c. Lender are not suspended from their license/operation
d. Obtain approval application form to loan radioactive material from AELB at the price of RM 1.00
e. form should be handover not more than 14 days from the loan date.
f. Approval (if qualified) will only be given for this period; whichever is shorter:- 
i) Period that is applied and cannot be extended; or
ii) 3 months for the first application; 6 months for second application; 3 months for third application for the same irradiating equipment; or
iii) Valid until license expired date; or
iv) Valid until RPO recognized valid period
g. Approval regulation is stated at the back side of application form...

What are the requirements and procedures to loan irradiating apparatus?

RADIOGRAPHY WORK AT PUBLIC AREA

What are the procedures to be followed to do radiography work at public area?
a. obtain approval application letter for radiography work in public area from AELB RM 1.00.
b. Form should be submitted not less than 14 days from working days and should include listed information:-
i) The start and finish location of the radiography work should be mark on the public area location map.
ii) Copy of approval letter/permit from local authority, police, contractor and others (whichever applicable).
iii) Dose calculation and the safe distance for exposed radiation not more than 2.5 µSv/hour.
iv) Define basis serial number, basis container and radiating apparatus which is going to be used
c. Items should be taken under consideration:-
i) Form should be approved/sign by OBTL/RPO only.
ii) Site evaluation will be done if necessary.
iii) Irradiating safety measures should be done before, present, and after radiography work done.
iv) Radioactive material should be started at storage area approved/recognized by AELB (if related).
d. Radiography work at public area only permitted between 12.00 am until 5.00 pm the next day but it will depend on AELB approval time by time. 
e. approval requirement will attach when the approval was given.

TERMINATION OF IRRADIATING WORKER 

What are the procedures for termination of irradiating workers?
a. Termination letter is handed 14 days before the termination by including complete details as follows:-
i) A copy of Medical Check up Resignation Confirmation Letter, page 16 LPTA/BM/5 section A.
ii) irradiating worker ID card (if applicable)
b. Medical Record Book LPTA/BM/5 Section A and LPTA/BM/5, license holder will be given a choice either:
i) Keep for own possession and will be responsible to make sure the record do not disappear [Regulation 45(5) P.U.(A)61] ; or
ii) Hand over to AELB to be kept.
c. if licensee terminate license, medical record should be hand over to AELB [Regulation 45(6) P.U (A)61].

RADIOACTIVE MATERIAL DISPOSAL

What are the procedures of radioactive?
a. Recognition letter should include all related documents: - :
i) Name of element and mass number, basis serial number and basis activeness
ii) Disposal method
iii) Permitted disposal methods are:
iv) Disposal/export to original maker,
v) Disposal at MINT, Bangi
vi) Other methods permitted by AELB.
b. After getting disposal permission and after disposal has been done, proposed the below information’s to AELB which are:
i) A copy of acceptance radioactive confirmation from the disposal area
ii) Ownership Statement, LPTA/BM/3 (for user)

DISPOSAL OF RADIATING APPARATUS

What are the disposals of radiating apparatus procedures?
a. Proposed desired letter which contain full details such as: 
i) Types and model of radiating apparatus
ii) Control panel serial number
iii) Disposal method
b. Disposal methods which are allowed:
i) contribution
ii) export / return to the original supplier
iii) Dismantle it to transform to old junk 
iv) Other ways which are allowed
c. After get a disposal approval and disposal has been done, proposed the below information to AELB:
i) A copy of acceptance radioactive confirmation from the disposal area or export document (if it is being export)
ii) Photo shows apparatus has been dismantle (if it is being dismantle) 
iii) Ownership Statement, LPTA/BM/3 (for user)
